We are currently seeking a Full Time LPN. The LPN practices under the direction of a registered nurse and provides nursing and facilitation services for assigned clients. The LPN on the Rehabilitation Team contributes to and implements evidence-based treatment strategies that are related to self-care and that promote physical, psychosocial, and spiritual health. The LPN also provides training to clients or family members regarding how to promote health, wellness and medical stability as well as to staff.   Hours: Friday 7a-9p, Saturday 7a-9p and Sunday 7a-7p.   Essential Duties & Functions - Timely identification of individuals in need of medical nursing services or interventions. We are searching for a Full Time Clinical Specialist to provide clinical management and consultation at ReMed’s residential programs.  Job Summary. This position provides clinical management and consultation based on training/expertise and brain injury education for brain injured clients.  This role works with treatment teams on clinical and organizational issues, assists with managing operational issues and provides staff training and development.   Duties include, but are not limited to:    - Attends team meetings and interfaces with treatment team to establish rehabilitation goals and coordinate treatment necessary to support individual client’s treatment plan. - Demonstrates the ability to support clients in difficult situations (e.g. refusal of activities, medications, and strategies).  - Works directly with the client and provides minimum to maximum assistance and/or supervision to promote the client’s success in achieving the identified treatment goals. - Monitors client’s treatment plan and progress towards goals, providing problem solving, conflict resolution and revision of goals/treatment plan as necessary. - Meets regularly with Director to update and review clients’ plan, progress and goals.  - In consultation with the Clinical Director, identifies, collaborates, and consults with physicians and other health care providers outside of the primary treatment team. - Ensures the ongoing clinical development of designated sites and systems including therapeutic groups, social activities and special events.  - Works to ensure implementation of governing policies and procedures (CARF, DPW, etc.). - Works to ensure the proper application of policies regarding hiring practices, disciplinary actions or termination, performance appraisals, EEO, anti-discrimination, and employee recognition. - Responsible for supporting orientation and training of staff in designated sites. Identifies staff training needs and coordinates resources to meet same. - Supports and enhances communication within designated sites (staff communication log, staff meetings, supervision and feedback, rounds, etc.). - Keeps staff updated of changes in company policies, procedures or other program/company/clinical practice related issues. - Participates in “on-call” system of coverage to support client care, staff, operations and clinical emergencies (includes holidays). - Works with operations to identify staffing patterns, clients and budget parameters to promote fiscal responsibility.  - Demonstrates a thorough knowledge of the budget guidelines including indirect vs. direct hours, food and recreation allotments and other program related expenses. - Monitors and ensures the timely submission of all required documentation by designated staff (e.g., monthly client billing session notes, log notes and monthly progress notes). - Ensures effective and efficient scheduling of staff in collaboration with Site Manager to meet client and program needs based on program budget, client hour grid, payroll grid, and staff competencies. - Maintains standards of cleanliness, maintenance and other health and safety related requirements. - Responsible for the clinical integrity of all clinical documentation through editing reports, developing and revising log notes and strategies.

The Clinical Director oversees and directs the development of all client treatment plans and programming for our Louisiana programs. This role is also responsible for the clinical supervision of staff and clinical direction of the programs.   • Analyze data to evaluate, revise and develop systems and procedures to enhance operations and clinical effectiveness of designated sites • Supervises or provides support to therapists in areas of specialty in other designated programs (e.g., supervision of psychologists for licensing hours) • Responsible to lead or supervise all groups in designated programs • Manages the performance appraisal process for RCM’s, therapists and provides input on clinical competency for all staff on performance appraisals • Constantly evaluate systems and procedures to look for more efficient and effective ways to run programs and develop other services and new program ideas • Responsible for program evaluation/quality assessment systems; tracking, completion and timeliness • Responsible for the ongoing clinical direction and management of each client's program • Direct provision of therapy specialty to programs when appropriate • Responsible for final version of all reports, including necessary signatures on assessments, reports and treatment plan • In residential programs, gives approval for 30 days of treatment for any needed clinical services (check approval through insurance carriers first) • Coordinates internal admissions (through transfers) and discharges; provides for carryover of needed systems and strategies • Coordinates beeper coverage with Program Directors and must be available for and notified of all crisis situations • Acts as liaison with family and funders to provide education, to document necessity of treatment needs, etc. Reviews all files prior to intake evaluations and makes final approval on all admissions in designated programs • Performs intake evaluations and writes report promptly • Responsible for overall assessment and development of the client's programs within two days after admission. • Participates and monitors completion of all program evaluation/quality assessment systems in a timely manner • Involvement in program development (who runs groups and trains/supervises staff in curricula used in groups) in designated programs and for the company as a whole • Assure implementation of other governing policies and procedures (CARF, etc) • Develop and monitor annual budget in collaboration with Program Director and other members of ReMed’s management team as necessary
