We are searching for a PRN Physical Therapist for our 8 bed residential programs in West Chester and Malvern, PA.  - The therapist will provide group and individual physical therapy services to clients in collaboration with an interdisciplinary treatment team to include psychology, physical, speech and occupational therapies. - This position also trains staff in any necessary techniques or procedures regarding client mobility and pedestrian safety status. - Specific areas of treatment may include, but are not limited to: vestibular issues, pain management, cervicogenic issues, flexibility and/or strength deficits, balance disorders, and development of home exercise programs.

This position performs examinations, evaluations, and provides treatment to clients with medical or health related conditions that limit their abilities to move or perform functional activities of daily living.   Essential Duties & Functions - Performs examination consisting of patient/client history, systems review and appropriate tests and measures. - Performs evaluation of clients’ mobility status and develops appropriate intervention strategies. - Implements treatment plans for addressing transfers, bed mobility, gait training, and program and/or community mobility. - Provides practical training in cardiovascular testing, individual fitness programs, use of exercise equipment and other physical development activities as appropriate. - Provides resources or recommendations to clients for appropriate adaptive equipment. - Provides practical training in higher order community integration activities and involvement in physical activities as appropriate. - Completes evaluation of prognosis and implementation of plan of care to provide interventions, conduct re-examination, modify interventions based on outcomes and development and implement discharge plan. - Utilizes treatment techniques to promote ability to move, reduce pain, re-store function and prevent disability which interrelate with other disciplines and supports the company’s philosophy. - Develops treatment plans to help prevent the loss of mobility to clients by developing fitness and wellness-oriented programs for healthier and more active lifestyles. - Diagnoses and manages movement dysfunction and enhance physical and functional abilities. - Restores, maintains and promotes optimal physical function, wellness and fitness, quality of life as it relates to movement and health. - Develops treatment plans to help prevent the onset of symptoms and progression of impairments, functional limitations, and disabilities that may result from disease, disorder, conditions, or injuries. - Provides direct and consultative physical therapy services, including assessments and treatments in accordance with regulation, standards, and guidelines defined by the American Physical Therapy Association (APTA) Code of Ethical Practice, State Practice Acts, Division of Developmental Disabilities (DDD), Health Insurance Portability & Accountability Act (HIPPA), Commission on Accreditation of Rehabilitation Facilities (CARF) and established company policies and procedures. - Plans and implements evidence based in-service presentations to appropriate audience as indicated. - Trains/educates families, funders, employers, significant others as needed to support the treatment and discharge plan. - Assesses and implements treatments for vocational/avocational pursuits by providing timely assessments for the vocational evaluation; providing task analysis for jobs as requested; developing strategies for the job. - Assesses, develops, and monitors, as needed adaptive equipment and/or compensatory strategies based on the treatment and discharge plans. - Responsible for teaching other team members strategies needed to support the client’s treatment plan. - Assists RCM and team in discharge planning process, assessing and securing needed services, modifying strategies for discharge setting, and educating caregivers. - Participates in team meetings and/or provides necessary information and interfaces with treatment team to establish rehabilitation goals and coordinates treatment necessary to support client’s treatment plan. Assist team in remaining focused on goals and discharge plan. - In collaboration with the clinical team, identifies and consults with physicians and other health care providers outside of the primary treatment team as needed. - Keeps current with professional certification/licensing requirements and participates in continuing education. - Completes therapy assessments and evaluation documentation according to established timeframes. - Writes monthly treatment plan reports according to guidelines and time frames and documents therapy sessions on session notes as they occur. - Records the accurate date and length of each client contact or service in the program file or monthly billing sheet according to program guidelines. - Prioritizes tasks to balance direct client treatment, staff communication, program responsibilities and written documentation.

The Shift Coordinator:   - Models appropriate service delivery for residents and mentors Behavior Technicians in the delivery of services to residents. - Communicates information to program management to ensure that appropriate staffing models are in place to address resident needs. - Fosters environments which create opportunities for learning and independence in daily functioning and interdependence within the community. - Provides training to Behavior Technicians as needed. - Leads groups as necessary, providing feedback and training to Behavior Technicians who lead or co-lead groups. - Facilitate accurate and complete data collection on the shift. - Assume the role of primary contact for clinical direction for the shift. - Perform clerical duties, such as taking inventory, ordering supplies, answering telephone, taking messages, and filling out forms. - Perform postural drainage, percussions and vibrations, and teach deep breathing exercises to treat respiratory conditions. - Work directly with the client and provides minimum to maximum assistance and/or supervision to promote the client’s success in achieving identified treatment goals. - Instruct, motivate, safeguard and assist patients as they practice exercises and functional activities. - Make modifications of treatment programs consistent with the established patient care plan - Monitor client’s treatment plan and progress towards goals, providing problem solving, conflict resolution and revision of goals/treatment plan as necessary. - Meets regularly with Clinical Director/Physical Therapist to update and review clients’ plan, progress and goals. - Administer active and passive manual therapeutic exercises, therapeutic massage, and heat, light, sound, water, and electrical modality treatments, such as ultrasound. - Observe clients during treatments to compile and evaluate data on patients' responses and progress, and report to physical therapist. - Measure clients' range-of-joint motion, body parts, and vital signs to determine effects of treatments or for patient evaluations. - Secure clients into or onto therapy equipment. - Fit clients for orthopedic braces, prostheses, and supportive devices, such as crutches. - Train clients in the use of orthopedic braces, prostheses, and supportive devices. - Transport clients to and from treatment areas, lifting and transferring them according to positioning requirements. - Monitor operation of equipment and record use of equipment and administration of treatments - Clean work area and check and store equipment after treatment. - Assist clients to dress, undress, and put on and remove supportive devices, such as braces, splints, and slings. - Administer traction to relieve neck and back pain, using intermittent and static traction equipment (as applicable) - Note changes in clinical status and summarize impact or effect - Identify specific elements of each of each intervention/modality provided to client, and utilize training/judgment to determine frequency, intensity, or other details in the plan of care - Provide summary/explanation of client response to treatment based on subjective and objective findings, including any adverse reactions to an intervention - Attend team meetings and interfaces with treatment team to establish rehabilitation goals and coordinate treatment necessary to support individual client’s treatment plan. - Identify staff training needs and coordinates resources to meet same. - Support and enhance communication through participation in staff meetings, feedback, rounds, etc.
